MVR Membership Application

To register please fill out our membership form

We require copies of the following documents. Please submit any and all that apply to your business:

• Valid Photo ID
• Valid Business License
• Ontario Incorporation Certificate
• Valid Tobacco Licence / Dealer Permit
• Current Business Number Document (HST)

We will process your application within 3 business days, and contact you via email after your application is approved.

Membership cards will be available for pick up at the courtesy desk, must present a valid photo ID.

Our membership fee is $20.00 annually.

Members are required to update their business documents every two years.
